Wow, all those comments on prevention of pressure ulcers aren’t much help when you are already dealing with one, eh?

I had one at 9yo, partly due to a sitting obliquity because of scoliosis, partly because I was prescribed a wheelchair with no special seating. I was stuck at home off school (with a visiting teacher for 2 hours 3x/wk) for 6 weeks which felt like years. I’ve had small minor reoccurrences 2-3x (I’m now almost 40) even though I take care of my skin and sit on specialized seating. Some people are more prone because of things like these. My most recent experience of skin breakdown was just because my skin got super dry this year (apparently a hormone thing for women my age).

Here’s what I’d suggest to help you cope:


  • unless you’ve had flap surgery, ask your doc how much time you can be vertical each day and save something special each day for that time. A friend visit, make a sandwich, wheel outside.... for me it was an hour each day, and I liked to use it to go for a wheel around the block or hang out with friends. It helps keep you sane!
  • ask if you can use a Ride cushion to be up and around a short time each day (assuming your ulcer is on your butt or sacrum?). There are Ride cushions or Roho cushions that float your bony areas so they don’t have pressure on them that you might be able to buy or borrow to minimize risk while you’re up. Your wound care team should have an OT who could help you find something.
  • meals: sign up for your local meals on wheels organization which in many North American cities can deliver a healthy meal to you once a day for a very low price. If you can afford it, try Home Chef, Chef’s Plate or another meal delivery service - some deliver full meals, others deliver everything ready to cook. If you’re in a city, try online ordering for grocery delivery from a local grocery store and choose a variety of skillet meals - they are healthier than TV dinners and only take 10 minutes in a frying pan.
  • connect with a local SCI organization and ask them for suggestions/programs for home bound people. United Spinal has chapters all over the US, and look up Spinal Cord Injury Canada if you are in Canada (fm me too if you are, I can connect you with the org near you).
  • sign up for a box/crate club like Escape The Crate it The Mysterious Package every month, giving you a puzzle to solve or mystery to uncover with clues each month.
  • watch streamers (if you’re not working). Twitch.tv and YouTube Live has lots of options. There are people gaming, cooking, building, making things, coding, illustrating, writing music, etc and it can be a good way to connect with a community of like minded people online that will help you keep the isolation at bay
  • get a Skillshare.com or Lynda.com account (a video course platform) often free through your local library and commit to learn a new skill while you’re healing. Having a target to work toward can help the time go by.
  • Reddit is your friend- I swear, you can stay occupied for months on Reddit.
  • there are lots of work at home jobs you can do that only involve a computer especially if you have customer service or call centre experience or know anything about computers. Amazon is known for hiring armies of home based web service testers, for example.

  • get a free account for Zoom or use FaceTime to connect with friends you can’t see while you’re at home. You can even watch movies with friends using Facebook now, which is a nice way to do something with friends remotely.

oh, and for daily life...

  1. exercises every day!! Even if you don't feel that great...sometimes it makes you feel better!

  2. Square up before you do ANYTHING. Sounds ridiculous, but I have to square up before I stand, sit, bend over, reach and grab something, friggin' eat, etc...keeping good alignment with core engaged for extra support will help your body keep things where they should be.

  3. Before I stand up, I move my feet around (draw circles?) a couple times, straighten both knees, and squeeze my legs together (my hips now dislocate inward and outward...goodtimes), all to align my joints before I put weight on them. I can't tell you how many times I've just stood up, willy nilly, and dislocated my knee cap/hip/ankle/toe, whatever. so now I just engage all my muscles to get everything where it should be and have many less problems.

  4. I have a serious of things I do ~ 4 to 5 times a day with the joints that fall out all the time. I straighten both legs and arms (get those knee caps/tibias and elbows back in socket!), pop my fingers, push by lower thumb joints back in, pop my neck, twist and pop my back, and check my collar bones. I've made it a routine and find that if I just check all the joints that fuck me up daily and cram them back in, I can often catch them before they cause more problems.

  5. If you can, get a memory foam mattress pad/bed. In my early 20's, my normal mattress would cause things to dislocate while I slept. I would wake up everyday with both shoulders completely out, and if I was lucky, totally numb, but usually intense shooting pain down both that would last almost all day. So I started with a 2" memory foam pad (found on overstock, but that was years ago...usually cheap this time of year!). I eventually increased to 4", then just said fuck it and got a whole mattress. My shoulders rarely dislocate at night now...however it's now my collar bones, ribs, and shoulder blades do regularly, but not a bad as with normal beds.

  6. I have two long and thing memory foam pillows..they're weirdly shaped (http://www.amazon.com/Rio-Home-Fashions-Memory-Pillow/dp/B004G8QCQS/ref=sr_1_90?ie=UTF8&qid=1324246170&sr=8-90 and the pillow with this set: http://www.amazon.com/Memory-Foam-Wedge-Pillow-Multifunctional/dp/B000PJBXMA/ref=sr_1_22?s=home-garden&ie=UTF8&qid=1324246263&sr=1-22...the wedge is nice too, but that little pillow is my favorite thing ever!) and smaller than most foam pillows. I take these with me everywhere. They really help me keep alignment while sleeping. Also, if I'm on vacation, they help me handle shitty mattresses.

  7. A lot of doctors will give you braces for your joints...while they're inflamed and hurting, that's useful to stabilize them until they're back to normal, but DO NOT wear them constantly! This will cause the joints to atrophy and dislocate more and more! So wear them when you need the extra support, but when they're back to feeling better, focus on strengthening!! I've had doctors tell me to wear them all the time and to not every lift any weights...both incorrect. strengthening (light weights and exercises) should always be the focus! Bracing only to heal and provide extra help when needed.

  8. Watch the weather!! My body reacts intensely to big changes in pressure. I used to live in the midwest and could predict a tornado as good as the weather channel. It's a different pain...it starts with an aching deep in my pelvis, then radiates up my spine and down my legs. From there my joints either: get so unbelievably lose and I dislocate fucking everything (~10 an HOUR...full body!) or incredibly tight and tense. I have since moved to the west coast, where the pressure changes are more subtle, and I don't have near the full body reactions I used to. Took me YEARS to figure out why the hell I'd wake up and my body would totally different...usually the weather (and food for some people). I figured it out by being a scientist about it and taking data points...a diary with daily joint issues, weather, food you've eaten should help you understand some of the trends and prepare for/avoid particular situations.

  9. Learn not to hate your body! I cuss at my bastard-y joints all the time, but when I dance, I <3 it! Find something you love to do, that is somewhat active, and learn to appreciate what your body is capable of doing, instead of focusing on what it can't do. For instance, I found that because my small muscles are constantly twitching to keep things in socket, I have amazing small muscle control. I can totally pop and lock because of it! (check out my vids! http://www.youtube.com/user/amirahfusion) I've invented my own bellydance fusion with popping and waving styles of hip hop. My silly joints have given me a special power! I hope you can find yours :)

  10. Try to stay optimistic!!! Look, I know it's hard with a degenerative joint disease...but we each can choose if we want to be optimistic or not. I find that having some hope/faith in science keeps me at least looking forward to my future, even though I know it will be filled with complications. I try to enjoy and be thankful for the now...what I can and am able to still enjoy.

  11. Also, medicinal marijuana over opiates for me! (getting my card in a week, yay!). I can't do normal pain killers anymore. Opiates make me puke EVERY time (and dislocate a variety of things while doing so) and I got an ulcer from NSAIDS. Honestly, pot really helps me not just handle my pain, but enjoy my damn life. It's better than being addicted to some opiates and out of it all the time. Since switching to pot, I have gotten a masters and Ph.D., as well as started my own bellydance club/troupe. I started enjoying my life again, which is so important!

okay, so...this was an amazingly fantastic weekend.

saturday: ended up going out for NYE after all - went to my favorite tap house with a group of friends. loved every minute of it; it's funny how when i feel like i want to be alone, what i end up getting more out of is time with my tribe. it was well worth the sleep i didn't get. :)

sunday: i will freely admit i was neither awake nor sober until about noon, halfway through our aquarium trip with brother-in-law and nieces (17 and 20), but we still had an amazing time. grabbed lunch afterward, headed over to clearwater to watch the sun go down and wade a bit, and then went back to their place to have some snacks and visit with them and their mom a bit more before we headed back. (well over 10k steps and i know for a fact i ended up at a deficit still, so, bonus!)

monday: slept for something like ten hours. hubby went shopping while i got myself cleaned up and did some chores (and enjoyed some desperately needed alone time; as fun as sunday was, it seriously taxed my introverted self and i was getting downright bitchy even despite all the sleep), i enjoyed the bottle of bubbly i'd set aside for NYE, we had meatballs with alfredo sauce for a late lunch/early dinner...it was just a nice mellow day.

so, today, life returns to normal. getting my food back on track and am going to do a leslie sansone video this morning, since it'll be my last for a few days; i have an appointment for a new hormone pellet insertion this afternoon. it's supposed to be okay to exercise within three days but i've found giving it an extra couple of days to heal is generally not a bad thing. and i can still lift and stuff.

fantastic news on the sleep front...i think i found an answer with a 7" wedge pillow. this, combined with a normal feather pillow, have resulted in snore-free, consistent, deep sleep over the last few days like i don't think i've experienced in years. i'm so happy i could cry, because i was getting desperate. i'll see how things go over this next week but as of right now, i can highly recommend it and have been telling everyone i know about it.

so that's about it. holidays were surprisingly excellent, and this was a fantastic start to the new year - i'm glad to be back at it. hope everyone has a good day today!!

While we always enjoyed doggy style we do a lot more of it these days.
Also, we picked up a wedge pillow (http://www.amazon.com/gp/product/B009HHLBKK?psc=1&redirect=true&ref_=oh_aui_detailpage_o07_s01) and it has been fun. My wife can lay on her back with it under her hips and it is great as it raises her hips and it changes the angle that I enter her and where I hit internally. It also allows us to maintain eye contact. ;) It's also a great position for me to give her oral as I can go for up to an hour without getting a sore neck.
She also likes to lay face down on it with it under her hips. Very similar to doggy but it's a lot easier on her arms.
We love to spoon but entering her while spooning is difficult now. We've compensated by spooning from the waist down which allows penetration and I can still touch her back, arms and face, maintaining a level of connection.
I think the final one that we enjoy is where she lays on her back and I lay on my side next to her, maybe at a 45 degree angle. She raises her leg closest to me, I slide my top leg under her raised leg and extend so that lies on top of or next to her other leg. This allows us me to enter her and get really deep into her.
Another one that we've added is where she lays on her side and moves her top leg forward with her lower leg running parallel to her lower leg. I then straddle her lower leg and slide into her. It's ok for us. I mention it as it's worth trying and I know experiences vary by person.

Oh, and one more thing... We added a Hitatchi magic wand and she loves having a non-penetrative vibrator.

what about a sex wedge? There are some cheap varriations on Amazon, like this one. It might help her open her hips.

This might not work for her, but what I thought of when I read your post was restorative yoga, which is basically about putting your body into poses that are supported by blankets and pillows and allowing yourself to open up your hips and shoulders. Obviously I don't know anything about her level of mobility, but if you think she could benefit from it, maybe this book and a commitment to help her with it might increase your intimacy AND help her become more flexible and comfortable.

I know the impending surgery is scary, but you’re doing the right thing seeking answers from people who have been through it. I wish I’d found this community before my surgery!

I have long hair and really like my hair, and I brought it up to my surgeon. He only shaved the bare minimum- about half an inch wide and three inches long. Even shortly after the surgery you could only see the incision if I had my hair in a high bun- a ponytail hid it pretty well and it wasn’t noticeable at all with my hair down. I’d mention it to your surgeon. One thing: if you keep your long hair, be really careful brushing it after the surgery. I draped a towel over my shoulders and bunched it up around the incision so I wouldn’t accidentally bump it while I was brushing.

I did have trouble sharing a bed with my spouse. I ended up sleeping in our guest room. Idk about the card, I’m in Texas and we don’t have them. But you definitely shouldn’t be considered “cured”, everyone says it takes a year or more to feel fully recovered from the surgery. Yes, when I started I had it - and it was severe. The first recommendation is in line with your added "note"...the full face works best for me, as it's passive air delivery (vs. something like nasal pillows) and offers a bit more space than a nasal mask.

The main things that helped me were (1) taping my mouth shut, (2) sleeping on a slight incline (this wedge pillow), (3) and sleeping on my back with a firm pillow under my head. Swallowing air was the worst for me when I was on my side, or when I was on my back and tilted my head to one side or the other. So, I conditioned myself to sleep on my back (it was a challenge because I prefer my side) and keep my head straight when I was on my back (like I was staring at the ceiling).

All of that and time help greatly. While you are adapting, take two Gas-X before bed. It's safe to take every night - it won't alleviate the problem, but will make it easier to deal with (lessens the severity and the pain).

edit - try to be mindful of your breathing through the day...if you are a mouth breather, condition yourself to breath through your nose. And, as an added benefit, the full face mask (combined with the heat/humidity from my line) really helped with my nasal congestion issues; if you need to, spray some otc nasal spray (I prefer 4-way) into your nose before you put your mask on at night.

edit 2 - it'll get better with time, and it will be worth it when you start sleeping good!

If a regular pillow isn't enough you can try a wedge pillow. They're firmer and kind of triangular shaped so it would probably do a better job of bending his body upward at the waist. I haven't tried it for this purpose, I only know about them because I use one for acid reflux (they help keep you up on an incline when you're sleeping which is good for reflux) although I think they make things like this specifically for sex too. This is the one I use which is cool because you can adjust it to a couple different heights.

Things I found helpful:
Ice Machine - You may be sent home with one, but find out. If you need to buy your own, I recommend it. ~$200 Brega Cube is what I got.

A way to elevate your leg. Pillows will work, this is what I got.

Shower Stool, non-slip shower mat if you don't have one. On the shower stool, I used an Ikea step stool we have and that worked well.

Ice Packs I got two of these. They get cold quick and last 20 to 30 minutes.

A charging station for phones, tablets, and other entertainment devices.

Other things, if you know who you plan to see for PT, talk to them to find out what you can do and should buy for home PT. I bought a balance pad, exercise ball, stretch bands, yoga mat, and put my bike on a trainer (already had). With this, I could do and still do some PT at home.

Other advice: Schedule your meds. You don't want to fall behind, the pain is intense. Drink a lot of sports drinks
or coconut water. Keeping hydrated will help.

Check out a thoracic wedge. I use it to correct thoracic tightness from computer posture. Lay on it and take 5-10 breathes then move it an inch. Dont use it below ribs or above the shoulders. Keep your lumbar spine flat (ie dont arch to allow your ass to touch the ground) and use your legs to vary the pressure.

Not sure it'll correct your issue, but it may offer some relief.

A peanut to MFR the muscles around the spine may also help get you some relief.

This is the wedge pillow I got but it was cheaper when I got it (~$25) - I think any wedge pillow could work, or you can get one that's designed for a single leg - just make sure you can use it with your leg fully extended.

This is the bed desk I got - I used it a lot for the first few days, but then I was able to move myself to a desk. I guess I should probably still use it when I'm on my laptop in bed but I don't... my laptop doesn't have any issues overheating and I feel like it just gets in the way sometimes.

You can cut these wedges in half and use them. You could also get yoga blocks that would work well. A lot of "speaker stands" are way more expensive than they really should be, so often it's best to find something else that works just as well. If you want to get actual speaker stands, make sure you measure and get the right height that you need.

I sleep on my back and I use this pillow to support my legs. It has really helped reduce the pain I get when waking up. Sleeping in any one position all night will make your body stiff, spondy or no spondy. So doing stretches and some warm ups in the morning help as well. Hope this helps!

I sleep sitting up. I could not handle sleeping on my side, and do not like the idea of the machine; so sleeping reclined it is. Not diagnosed because that costs monies, but I do snore very loud, and wake up tired. Lately I have been waking up in the middle of the night to use the bathroom which can be a sign as well. Sleeping reclined has helped quite a bit. I feel better during the day and do not feel like I am going to fall asleep at work or in traffic.

I will be picking up one of these to help eventually.

I either sleep on my side with a body pillow between my knees and supporting my up side arm or on my back with a special knee wedge pillow. It’s slanted on the side that goes under your knees then is flat. It keeps me from hyperextending my lower back (or hyper flexing? It keeps my lumbar and sacral spine flat and also keeps my knees from falling open and making my hips want to sublux). It may be awkward at first but check out a wedge pillow. I had some bad reflux and it’s pretty much gone now with diet changes and the pillow. It’s cheap enough to give a try. See link below

Laying down flat will be quite painful. A bed wedge made a huge difference in managing the pain and being able to sleep. No BLT - bending, lifting and twisting. Good luck with the micro-D!

i would add that, while there is a physiologic basis for smoking and alcohol cessation, weight loss and trigger food avoidance, the only life improvement modality that's ever been proven to work by evidence is raising the head of your bed. if you cant put bricks or cinder blocks under the head of your bed, buy a wedge pillow for 30 bucks from amazon.com

For the night issues, I recommend getting a wedge pillow I bought this one and really like it. Make sure you have one that's the length of your torso, otherwise it'll fuck your back up, like this one that I bought first and returned. Or if you have more money you might consider this type, it helps to prop your feet up too. I also slide down the bed if I'm inclined without a pillow behind my knees.

I find it's best to eat about 2 hours before bed. If I eat too long ago my stomach stays empty for too long and bothers me, but if I eat before bed it hurts more the next day.

Talk to your doctor about getting anti-nausea pills. I use zophrin 4mg sublingual melts and they're super helpful. Ginger helps some people, but it fucks up my stomach so I can't consume it.

It might be stress. There's nothing wrong with that, mine was caused by stress too. Stress is a chemical process, it raises cortisol levels which impair digestion. My doctor explained it as your body going into fight or flight mode, which shuts down unnecessary things like digestion while there's more important things, but you can't tell your body to start that shit up again so it becomes a chronic problem. Regardless of the cause it's still a medical condition, it's like dismissing someone with a broken leg because they did something stupid; it doesn't matter why you're in pain, your symptoms are still real and still fucking up your life.

I wrote up a bunch of different things that will be easier to eat in an old post, look through that and try to shift your diet more towards things that won't cause problems.
