You can definitely recess the medicine cabinet. If you are gutting the room to the studs you can get a larger cabinet (wider than the typical space between studs), but you may need build it out like a window (adding jack studs). Go with low profile fixtures. Here is a nicely done small bathroom. I think the rectangle sink is nice, gives you a large sink area while feeling modern and open. The spout and handles are thin and simple. The toilet is simple. The floating vanity give the illusion of space because your eye can see under it.
Niches in the shower: make more than 1 and make them bigger than you think. I also would put them on the far wall across from the shower head instead of the side wall so you don't see all of your clutter and shampoo and razors when you walk in.
And then across from the sink some sort of wall towel storage. Not that style, but something with a shelf to keep an extra set of clean towels, hooks for the towel you are using or a bathrobe, and the bar allows for hand towel to use for the sink.

It'd be relatively easy to get ahead of this if it comes up again by storing your controlled meds, in their original containers, inside of something like this or this. Keeps medications away from people who shouldn't have them, can be bolted into a wall, cabinet or drawer and looks relatively secure otherwise. Just make sure to dry your razor well before putting it away as it doesn't seem like it gets much airflow.
To tack onto this it might be wise to keep all meds in a locking medicine cabinet that only you and your DH have the keys to. Not only will that prevent any accidents with your LO, /u/whtbrd, but it will also keep any guests- wanted or unwanted- from shopping around in your medicine cabinet [that way they can't steal any or tamper with any]. If anyone other than you or DH need something out of the cabinet either you or DH will go get it for them.
Poke around on amazon for a bit. It doesn't have to be something like this either, it could be something more like this, this, or even this or this as long as it locks well enough to keep unwanted people out. It doesn't need to be fancy. Don't even bring up the fact your worried that she might tamper with your meds to your DH, just tell him that your locking all the medications in the house- including yours- up so LO can't get to them and show him where the key is going to be kept.
For my small bathroom I made a full mirror wall and added storage by buying two more of these. Hand towel on the bottom bar of that shelf for appearance and for guests. Something to fill that space under the oddly positioned mirror - maybe some interesting stick-on and removable backsplash tiles? I bought these but you may want to get something with more color! More in terms of color - maybe something to hang on the left wall.
Once you clear the space over your toilet, maybe a decorative tissue box, fake plant, or anything else decorative. Definitely a towel hook. YOu dont' want to keep hanging that over the curtain rod.
