Apple recommends you calibrate the pedometer on the Watch by going for a run with it and the phone once, then after that it's able to guess distance in the same way a pedometer would. Your friend will be able to listen to music which is stored on the AW if they have bluetooth headphones. They'll be able to use 3rd party workout apps as long as they don't require a phone connection (some do, some don't). The built-in workout app will work great too. Your friend will be able to connect a bluetooth chest heart rate monitor to the watch if they want more accurate HR and calorie data. The built-in sensor is okay for running but not as accurate as a chest strap. They obviously won't be able to use more sophisticated running apps which map your run using GPS and elevation with a barometer. They will be able to look back over past runs and see stats on distance, average pace, average heart rate, calories, and duration in the Activity app, but they'll only see one workout at a time. The Activity app doesn't compare past runs or show graphs of improved pace like more sophisticated apps will.

I like to run with my iPhone so I can use Runkeeper, take pics, have more music to choose from, and have a phone just in case I need one. I have this runner's belt which keeps it snug against my lower back and I don't even notice it. So that's something to consider, but it's not necessary.
