If the not falling out factor is your main priority you need Bose IE2s. I've been using them for four years and have never once had them fall out of my ears (even push out a little bit). They make a water resistant version - SIE2s - although I don't think its worth the extra cash. You are paying more for them because of the name, and they don't sound as good as other traditional $100 headphones, but I still strongly recommend them because of the perfect fit.

Sennheiser and Adidas made a bunch, one of them was the cx685s. I had the cx680s (which had a mic) and they sounded pretty shitty, but apparently the 685s are an improvement (at least in comfort as I've read) only thing is there's no mic. Mine did tangle alot too. But they're cheap.

Bose has the SIE2s, which have a mic, but are pricier. The reviews should give you an idea on how they sound (never used them).

Lastly, Monster makes some. Not cheap, and one reviewer said there were some fakes being sold through Amazon so if you buy them, make sure they're genuine or buy from Monster. One thing to note, Monster has shit customer service. Dealt with them before on a pair of broken Turbine Pros and it wasn't very easy.
