If you look closer it says "Peter David Inc." and "Made In China" with a phone number "(619) 429 0901".

After a quick search on google, it is linked to a 1990 source :

So I think the template comes from "Peter David" and it's manufactured in China. Peter David seems to focus only on baseball team but maybe they can do some freelance from time to time ?
