It's a Lukas Blackbox, model LK-7900 FHD ARA. Possibly this although it was a gift so I can't be 100% certain. (That link seems to mention all of the features I'm familiar with, though.)
Also, if anyone is looking for a quality dashcam, this is one I got. Been using it for about a year and a half now. Awesome camera.
