PHP: The Right Way is an excellent option.

Another one is PHP & MySQL (https://www.amazon.com/PHP-MySQL-Development-Developers-Library/dp/0321833899) It has just been updated to the 5th Edition.

The 5th edition is the most recent, I believe, which covers up to PHP 7.

I learned by checking this book out from the library https://www.amazon.com/PHP-MySQL-Development-Developers-Library/dp/0321833899/ref=sr_1_2?ie=UTF8&qid=1493684388&sr=8-2&keywords=php+web+development and then built a web application.

Edit that was 11 years ago, no idea what this book looks like now. But it worked for me.
