With your situation it does seem important for you to find a good gastroenterologist who should ask you to provide a stool sample for analysis so he can find out what the hell is actually going on in your digestion. I picked up something in Tanzania years back and had similar symptoms to yours. After failing to fix the issue over and over again, a stool sample analysis was finally the only way we ever figured out exactly what was in there and how to treat it (it was some rare bacteria or something like that). This seems important in your case.
