This is from the ASOIAF Miniature Wargame, more specifically the Stark Heroes box #1 . The game has a lot of great art and is 100% book only inspired.

Osha, Rickon, Shaggydog, Meera, Jojen, Syrio and Arya in Stark heroes 2

Theon (Reek), Ramsey Snow, Roose Bolton, Varys, and Littlefinger in Neutral heroes 1

Pycelle, The High Sparrow, Tywin, Tyrion and The Hound in Lannister box #1

I can go on. There are atm 4 factions (+ neutral faction), three different core sets, about two dozen units and three additional hero boxes than listed above. Feel free to PM me if you have questions about the game itself.

Yes, a very big difference. They're totally different formats. The novellas are written books (though A Knight Of The Seven Kingdoms does have illustrations). The graphic novel is a comic book and, true to its format, focuses more on its art than prose and dialogue.

If you're looking to get into them I'd suggest reading the actual novellas first, then checking out the graphic novels. A Knight Of The Seven Kingdoms is an excellent way to read them (it includes all 3 novellas Martin has published thus far), the illustrations within it are top notch:
