The bigger the better. It isn't strictly necessary with a laser mouse, but it helps to demonstrate the optimal configuration.

Also, the 2 feet part isn't strictly necessary for MWO, since it limits pitch so much already, but it helps tremendously for other games that require more vertical movement.

It did scare me that it looked plastic and cheap, but I was pleasantly surprised once I got it. It feels good, it looks good, and it works well. I do wish it had another two or three buttons, but I got a Razer Naga Hex to fill in the gaps. The 8-directional hat could fill it out, but I find the diagonals difficult to hit (not willing to put in the practice or risk hitting the wrong thing).

Found it, it comes with a hex wrench to adjust it, easily adjustable to left handed. Joystick left hand and mouse right seems to be the way to go.

I use a SteelSeries 3Hv2. Decent price, the audio quality is pretty good, and it's 3.5mm because eff USB audio drivers.

I'd back this up. I've owned the G5 and currently own the G500s. The 400s and 500s are pretty similar, with the 500s having a higher DPI (4000 on the 400s up to 8200 on the 500s IIRC), two extra buttons, and the ability to add extra weights. Both right in your price range.

logitech G430, 7.1 surround sound sound, good mic quality, comfortable (over the ear means that it does not press your ear into your head, it is actually over the ear), wired with the wire quite long, but $58 on amazon right now so slightly over your budget.

Stand up mics are less ideal. they pick up outside noise, especially if you have a TV or music playing in the back ground.
having the mic next to your mouth really improves the fidelity.
I hate playing with people who have stand up mics. you can really tell

If you are only using the headset for voice chat, I like to use something like this

I was getting pretty freeking salty today when MWO drops frames while streaming, but every other game I stream has zero problems.
