Looks like a lot of scaling/dry skin built up.

While you have the ketoconazole shampoo on your head, scrub with something like this:

https://www.amazon.com/Shampoo-Scalp-Massage-Brush-Brushes/dp/B00SNF1O90/

If there's a cheaper pack grab it, but get one that just like that. The ones with the big squishy cones in place of bristles won't scrub well enough.

Keep using the medicated shampoos. Scrub at least every other day until the scale is gone, just don't scrub too hard, you don't want to break your skin. It might be worth using the 2% shampoo more often until your scalp calms down (assuming it isn't to harsh for you, everyone reacts differently of course).

I wouldn't use conditioner since virtually all of them have bad ingredients, and are likely too occluding. Maybe someone else has a good recommendation.

You could also try pre-washing with a sulfur-based soap. I've recently started using one for the dry flaky skin on my face and under my beard, and so far the results have been great. It really helps break up the flaking skin/scaling, and does a very good job of removing the oils. It might be too drying for you, but it might be worth trying. I got this one, though I'm sure other brands are just as good:

Hey man try "CeraVe SA Cream" https://www.amazon.com/dp/B00D7BZNT2/ref=cm_sw_r_cp_apa_Ao5MAbD837WBR

You can also find it at your local CVS or Walgreens.

I just started using it for 5 days now and it cleared up my dermititis almost completely on the sides of my nose. I've been applying it once at night after splashing my face with water and it does wonders. At first the flakes didn't go away but on the 4th day after washing my face in the morning and seeing the annoying flakes visible after I went to try and pick some off and instead of them being hard and crusty like usual and painful to peel off, they came off very easily and gently to reveal healthy skin underneath. It's amazing. I'm just hoping it'll keep working.

I think what makes it work so well is the Salicylic Acid which helps to exfoliate the flakes and hydrate your face.

Hope you give it a try and it works as well for you as it did for me! Because your dermititis looks very similar to mine so I have hope!

P.S. shaving also helps a lot too. And try using an electric razor to not irritate your face.

Im also using pure honey, it has to be 100% raw honey, basically straight from the hive and put in a tub/jar

I use Maunka Honey, every second day use it as a face mask and leave it for around 3 hours, and then every day inbetween use it as like a facewash/cleanser but leave it for 30mins-1hr

basically you mix it with water, make sure to natural water from a bottle from the shop, since people say there is stuff in tap water like chlorine that will affect it

and best way to wash it off i think is if your doing it before you go for a shower, dip your face in the sink ( cold water ) , some of the honey will dissolve off, but the rest use the tip of your fingers gently to get the rest off or any dry/crystallised bits and then go for a shower after

since sitting under the shower trying to get the honey off will make your sub derm more red and irritate same with the pressure of the water if its a good shower

I use this stuff, little expensive but its pretty good

I like this one cause its a bigger jar, i would get the UMF 10+ one but its a smaller jar and more money

You’re very welcome! At the beginning I was using it every day for probably around a week just to get everything off the scalp and keep it off. Now I won’t lie I’m pretty lazy so only use it every couple of weeks. I do get some scales when I go that long but they can be easily covered by my hair so I don’t bother to use the ACV until it starts getting decently bad again. I forgot to mention, I also use this scalp massager/shampoo brush: https://www.amazon.com/Scalp-Massager-Shampoo-Brush-MAXSOFT/dp/B074ZDXFL6/ref=mp_s_a_1_3?crid=2PAHXQHHHATTL&keywords=scalp+scrubber&qid=1555468179&s=gateway&sprefix=scalp+s&sr=8-3 and it also helps keep some of the flakes away as well! If you have any more questions please let me know 😊 I hope you try it because it’s saved me from going to the dermatologist again and constantly refilling prescriptions for medicated shampoo or buying expensive shampoo that doesn’t really work.

I do. This recipe doesn't work on my scalp. I haven't figured that out yet. I'm not sure of the brands but once I get off, I'll look at what I use and update you. I get in the shower, rinse my body, then lather body wash on me. Once i do that, I use 2% zinc oatmeal soap on my face and rinse my body off. After I rinse my body, I immediately rinse my face. Now my pores are cleaned and I keep a little Tupperware with baking soda that I dab my my finger tips in to and pat my face. My hand and face are wet so it sticks with no issue. Little dabs are the trick, don't rub it in because it'll scratch you. Let sit for 2-3 minutes and rinse off. Pat your face with the towel and then dry off. I moisturize once I'm out and a little goes a long way. Also, it's some SD specific moisturizer that I'll update you with when I can. I really don't know the science behind all of it. I'm almost 27 and have had this issue since I was 14. It's really been a trial and error situation. This is the only thing I've done that's worked for more than two weeks. Pardon the grammar, I'm on lunch and just trying to outline my daily routine. I'd like to add that there have been days where I don't do the baking soda, and I don't immediately break out. I try to do it every day but my world doesn't shatter if I miss a step here and there. I hope I am helping and not just rambling.

Edit: moisturizer- DermaZinc Cream - 4 oz https://www.amazon.com/dp/B0000ZHOH2?ref=ppx_pop_mob_ap_share
Baking soda- Sodium Bicarbonate (Baking Soda)... https://www.amazon.com/dp/B0753YZ3FS?ref=ppx_pop_mob_ap_share

I hope these links work. I just checked my Amazon orders and shared the links it gave.

Hello and welcome to SebDerm you are in the right place.

First and foremost : stop buying new products every week, unless you have desastrous effects or flare ups.
Introducing a new product in your skincare routine means that you must use this product dilligently for 3 to 4 weeks before you can draw any kind of concrete conclusion.

You mentionned that Dandrazol works for you that's very good, hang on to it- use that frequently for a few weeks with a type of oil as suggested below for at least 3 weeks before you draw any conclusion.
If you have a big problem with flakes I strongly recommend one type of oil for your scalp and behind your ears:

  • MCT Oil.
  • Mineral Oil.
  • Squalane Oil.

PS: I'm personally a fan of MCT OIL for the cost efficiency I used this one in the past but do your research and try to find a product yourself.

If you want to relieve the symptoms on your face and still keep it budget friendly :

  • Benton - Aloe Propolis Soothing Gel - use this after washing your face - Generally Safe contains Propolist with is an antifungal and helps inhibiting Seb Derm.
  • Hada Labo Rohto Gokujyn Hyaluronic Acid Cleansing Foam - Might cause a reaction depending on your skin or might work
Note if you use product or a similar one I recommend that you use a suncreen from this list the budget friendly Japanese products are not available on Amazon prime anymore but you can generally find them on Ebay UK.

For lasting improvements inspect your routine most people have bad habits and bad habits die hard.

  • Do you use any make up ? did you notice any patterns of flare up when using some type of make up ?
  • Do you smoke ?
  • How is your overall diet ?
  • How is the quality of your tap water ?
  • How often do you wash your pillow and bed sheets ?
  • How is the moisture in your living space ?
  • Do you have good sleep habits ?
  • Do you lack any vitamins when did you last check your blood or do a full checkup ? Did you go see a dermatologist ?

If you have any questions and can't find the answer by yourself don't hesitate we are here to help you fight this stupid skin condition !!!

Ready for a book? Here you go!

I’ve suffered from seb derm AND acne prone skin since puberty, and I’m 28 now. If I was still using my original Reddit account, you’d see a post or two in my history from years back when I reached out to r/SkincareAddiction for advice. Lots of good info there, but unfortunately, none of it was very effective for me.

Quick overview of my symptoms/problem areas before I tell you what’s been working for me for the past five years:

  • When untreated, 50%-60% (and at its worst, up to 80%) of my facial skin is red, rough, and flaky—basically in a “flared up” state, but permanently. My scalp is another beast entirely; most of is covered in bump-like clusters of flakes, which become easily visible and look like a snowfall of yellow-tinted dandruff.

  • Particularly affected areas include sides of nose/nostrils, center of forehead, eyebrows, sides of mouth, hairline, and ears (outer and within the start of the canal)

  • Makeup application is next to impossible in this state, and it is physically uncomfortable.

Believe me when I say I’ve tried just about -everything- that someone with a decent income living in the US can get their hands on. I’ve even gone so far as to buy and try a product discontinued in the US due to FDA regulations via Ebay, shipped from Russia. No luck there, either.

Now, my case is rather severe. I don’t get “flare ups”. For whatever unlucky reason, this is just how my skin is. I’ve yet to discover any underlying chemical cause, but I’m still working on it.

For now, and for about the last five years, this is what has worked for me:

  1. My #1 product is something that most people who are serious about skincare will warn against the regular use of, but in my case, it’s a necessity: a topical prescription steroid lotion. I use hydrocortisone 2.5%. Without it, the rest of my skincare might as well not matter at all. When I apply after every shower (every 2-3 days), my skin is virtually flake-free, and will stay that way for some time. I can wear light foundation and powder and it looks good. I’m serious when I say that without this, I’d be miserable, and I’m sad I went so many years without it.

  2. Mild cleanser. Don’t get something harsh, and try to avoid most “natural” products, be they homemade or retail. A lot of natural cosmetics contain ingredients that can “feed” and essentially encourage the fungal aspect of seb derm.

  3. Physical exfoliant. Be careful with this. It’s tempting to regularly scrub the hell out of your face to get rid of all the flakes, but you don’t want to cause excessive micro-tearing; it’s bound to happen, but you can limit the severity by being extra-gentle. I use a salt scrub from Lush (the only “natural” product I use), but immediately follow up with my mild cleanser to limit my skin’s exposure to the stuff the seb derm likes to chow down on.

I have used a Clarisonic in the past with my mild cleanser, but I got tired of having to constantly disinfect it.

  4. An oil-free, fragrance-free moisturizer. After I apply my steroid lotion and let it soak in for a minute or two, I apply about a nickel-sized amount of moisturizer. This keeps my skin feeling hydrated and helps to replenish some moisture-loss from the steroid cream.

To break it down in order of steps:

  5. Exfoliate in shower with salt scrub.
  6. Cleanse with mild cleanser
  7. Immediately after patting face dry, apply a little less than a dime-sized amount of steroid lotion.
  8. After steroid soaks in a little, moisturize.

Now, for my acne issues — I get a ton of blackheads. I think it has to do with the overproduction of oil thanks to the seb derm. All I really do for it is, about once a week, gently extract after a shower when my pores are relaxed from the cleansing and steam, using a metal blackhead extractor. If you choose to do this, be very patient and careful. I caused some pitted scarring in my cheeks when I used to just go to town. Skin with seb derm is extra fragile, and the steroid lotion causes some thinning over time.

Secondly, before my other products, I swipe a salicylic acid pad over my problem areas. This has really made a difference.

Whew! That was long. I’m just really serious about sharing my experience with people who have to deal with this incredibly annoying and often-times depressing condition. I hope this helped. Feel free to ask questions! I’m an open book when it comes to my skin.

TL;DR: This is the moisturizer I’m currently using:

Neutrogena Oil-Free Daily Facial Moisturizer for Sensitive Skin, Ultra-Gentle & Lightweight Moisturizers Free of Fragrances & Dyes, 4 fl. oz https://www.amazon.com/dp/B000052YOX/ref=cm_sw_r_cp_api_i_xtbXCbHYAB6TB

Two other good options are just a basic Zinc Pyrithione shampoo (make sure it’s sulfate free) or sulfur soap. I found the Ketoconazole was just too much for my skin, and washing daily with a much milder soap helped a ton!

It’ll take time to heal - mine took almost two months! I also made sure to keep my shower water filtered in case I was getting buildup on my scalp.

Noble Formula 2% Pyrithione Zinc (ZnP) Original Emu Bar Soap, 3.25 oz https://www.amazon.com/dp/B00G7TUDA0/ref=cm_sw_r_cp_api_i_LmVwDb22285HH

La Roche Posay makes several Anthelios sunscreens. I checked the ingredients in this one and, unbelievably, it looks like it doesn't contain any bad ingredients for SD.

I hope this works, I'm going to try it. Ingredients also have something called cassia alata leaf extract which looks like it has antifungal properties.

I don't know anything about this, so I'm in the same boat as you, but thought I'd post this option if you'd like to try it too.

Thanks for asking this, it got me researching! :)

Yes, look up "vanicream free & clear shampoo"--- my dermatologist recommended it to me.

https://www.amazon.com/Vanicream-Clear-Shampoo-Sensitive-fragrance/dp/B0006FMK98

It has no SLS/paraben free and all of that good jazz and it uses the same active ingredient as H+S (Pyrth. Zinc).

I heard this is really good, it’s on my shopping list. But I’ve not tried it yet. I’ve sebderm on my eyebrows and it definitely calms the area. You can also try the fresh aloe Vera plant which I’m using now.
I’m also trying out raw honey mask recommended by here few days ago which you put on three hour per day. I know it’s a long time but I don’t know if there’s any use yet. But it’s very itchy during the three hours. I managed to put it through tho.
I also take supplement which is probiotics and fish oil which has omega 3 and it also helps!
There’s a post about how she treats her sebderm with lotrimin ultra. I also got it yesterday and hope it works.
So my routine is:

  • Honey mask at night three hours
    -am: aloe Vera
-pm: aloe Vera underneath lotrimin ultra

Remember to stay away from hydrocortisone cream. I learn my lesson the hard way even I only used it for three- four days. Immediately got a super bad flare up after I stopped for 2 days. My another dermatologist told me Eyebrow and eye area are extremely sensitive.
I hope it helps. I’m also battling with sebderm.

It's always possible, but I think that's pretty rare. If you want to be 100% sure, buy this stuff(can be found in many grocery stores) and do a spot test.

https://www.amazon.com/Astroglide-Waterproof-Silicone-Personal-Lubricant/dp/B002DUDJWG/ref=sr_1_1?keywords=astroglide%2Bx&qid=1563719869&s=gateway&sr=8-1&th=1

It's only made of 2 ingredients, one of which is dimethicone.

EDIT: Yes, I'm serious BTW. You can zoom in on the bottle on Amazon and see the ingredients list for yourself.

I've tried 25+ different fluoride free toothpastes and none of them were that great. Finally found Dr. Sheffield's Peppermint and I've been using it ever since. I'm not sure which one you like, but if you're looking, this one has been the best by far.

I use a zinc soap bar. They are a bit expensive, but gentle on my hair and work very well. I can't use shampoo, but I needed a way to treat my scalp. I do a rotation of this and baking soda/bentonite clay. I'd say I use the zinc soap maybe once a week on my scalp.
