There are always a few problems that get students in grammar and writing (I'm a teacher/writer myself). To improve your grammar you must demand DEMAND that everyone correct all of your errors all the time until it becomes automatic. This sounds awful, but it will be worth it, especially once you get to the professional or college level.

To improve your writing on your test you need to: taste your title, use ST words, use your strength, paraphrase a friend, and make meaning.
Taste your title to your writing means to go and use an action/description word in your title to your writing tests. Instead of writing "My Life in English" write something like, "Alive and English" or "On an English Stage" or "Terror and Good Times". Any of those brings in an image and a feeling to your writing; do one, and you'll find your marker immediately gets a favourable impression and gives you a better score.

I can't go into huge detail here (it would take too long), but there's a whole book about it that guarantees to improve your writing score 10-25%. It's on amazon and costs like $3 for an E-reader copy. It's called "Write Better Right Now: An ESL learners guide to acing the toefl". I wrote it after 8 years of teaching ESL and it works. I'm not sure if I should post a link here (https://www.amazon.ca/Write-Better-Right-Now-Instantly-ebook/dp/B071GDJFKD/ref=sr_1_4?ie=UTF8&qid=1517204562&sr=8-4&keywords=write+better+right+now) , but try the exercise I've suggested and read the first few pages of the book for free to get a feel for it. Hope it helps!
