I use the Vivtar DKS-25. I did some modifications to the outside, but I fit a ton of stuff in there. It also has a built in rainfly, and can be used as a sling or backpack, etc. I took off the logo, sewed some butterfly loops and additional straps for the tripod and a few other things, but overall it's pretty big without feeling bulky, and the price is nice. Tons of pockets, customizable bottom area, side access for the camera, and a large upper compartment is what I was looking for. I hate rear-access bags. Also, there is a laptop compartment that can double as a water reservoir pouch, or to put extra padding in, etc. I'm really pleased with it after trying several other bags. I'm not sure how big your respirator is, I have one but usually just put dust masks in my bag and take a respirator separate if needed. However I have lenses in there, first aid, paracord, filters, walkies, flashlights, you name it.
Go for the 3M 7500 Half Face series. It's silicone so it gives a really comfortable fit over long periods of time and make sure you buy the correct filters. Here are some links:

By far the most important part for me is a decent set of gloves. Personally, I use these and I opt for fingerless since I do a fair bit of climbing and I need the grip but they have fully fingered for sale as well.
You don't need a super expensive flashlight. Most of my explorer friends use the small Ultrafire Cree lights. They're cheap, reliable, and ridiculously bright for the size. Buy several of them and keep batteries with you.
I used this mask. It's pretty cheap, it fits really well, and it doesn't even look that dorky.
Doesn't come with filters since there are all different types, but the 3M filters it takes are very common and with a little research, you'll be able to find the correct filter rating for what you're doing [e.g.: Asbestos, CO, lead, mold, etc...].
Maybe one of the lower end Sony E-mount cameras?

I've taken photos with a Sony NEX-3N, but the built in flash alone didn't quite cut it in dark areas.
