The ACG model is the shit you will never have to buy another set if you get the ACG, but I think its overkill if you just have one dog. The 20-30 dollar sets without the removable blades etc they wont drive through the paw hair which gets matted. Also pick up some Cool clip can. One can can last for years. The blades themselves don't get too hot but the cool clip keeps the whole thing lubricated during the clip. One set of blades can last about 2-3 years or about 12-20 clips.
As far as the pack leader comment and in our experience your dog will probably attach to one of you. You will notice that he/she will always want to be in the same room as you or your spouse. Some people find this annoying and some people like it. It probably differs by owner but in our experience and others with similar spaniel breeds they report some of the same thing.
It’s called veterinary clinical care

When I adopted my cocker spaniel she had already gone deaf from frequent, untreated ear infections. We went to the vet probably 4-5 times in the first 3 months due to recurring ear infections. My vet recommend this PhytoVet ear cleanser and it has seriously worked wonders. It’s been 9+ months since my pup has had an ear infection. I use this 1-2x a week (especially after baths). A bit expensive but pays for itself considering the cost of a visit to the vet + antibiotics for an ear infection is twice the price of a huge bottle. Went back through my amazon purchases, here’s the link if you’re interested!

I fostered a cocker for a few months and we trained him to use bells attached to a string on the door. It sounds like he knows he needs to go out, but maybe hes just afraid of the doggy door?
Amazon has them but for less than half of that you can go to your local craft store like Michael's and buy a yard or two of ribbon and some bells and make it for your self!
It's a long process but the pups I've trained to use this method pick up on it quickly! You just lift their paw and tap the bells each and every time you take them out. That way they have a way to alert you that they need to go out!
He might just be scared of the doggy door, and hopefully this helps give him a way to say he needs to go!
My cocker spaniel (10 months old) has a ton of allergies (we are in San Diego California, where the desert air can cause a ton of allergies.) She's had antibiotics for infections from allergies on her skin, medicated eye drops, and medicated ear drops. To try to prevent that she gets allergy shots and is on Zyrtec allergy pills, and we have an OTC eye rinse we use daily, as well as ear drops to clean her ears once a week or so. I also supplement her diet with fish oil and it keeps her skin less itchy.
Here's a link to the eye rinse we use. We get it on amazon. Super cheap, lasts a couple months. Nutri-Vet Eye Rinse Liquid for Dogs, 4-Ounce https://www.amazon.com/dp/B0018CE84I/ref=cm_sw_r_cp_api_zboJzbVWX14PX
This is what we use for Blondie: http://www.amazon.com/Virbac-3008-Epi-Otic-8-Ounce/dp/B002CZT12O/ref=sr_1_1?ie=UTF8&qid=1370276864&sr=8-1&keywords=Epi-Otic
Sometimes the vet will gives us an antibiotic ointment to use along with this and it clears it up pretty well! I use this stuff every other day to clean her ears.
I've only tried this one, but it works for us: https://smile.amazon.com/gp/product/B001FTXPO8/ref=oh_aui_search_detailpage?ie=UTF8&psc=1
