It's easier to push heel-to-toe with a very low board (like you see with Adam and company). This is much easier to do over long distances, but toe/ball pushing is best for sprinting.

I used to get that knot you're describing since I have flat feet and used to have to stop pushing after a mile or so. Then I decided to get the SuperFeet Orange the long Treks guys endorsed and swore by, and now I never have that crazy knot pain. Went 130mi in 2 days with them and never had any arch pain.
