Failure Is Not An Option has been on my to-read list and is written by NASA's flight director during the Apollo missions.
Dennis R. Jenkins' "Space Shuttle:The History of Developing the National Space Transportation System" is a must read. So much insight into how the shuttle became what it was, and it's fascinating.
