Sounds like it might be vulvodynia. I have it (and its cousin, vestibulodynia) along with my vaginismus, and it sucks hard.
Before you see the dermatologist, it might be worth it to try some of these things:
*use milder laundry detergent and body wash (but DO NOT scrub your vulva)
None of these will cure you, but it might help a little. Check out r/vulvodynia if you have more questions. I also recommend the book When Sex Hurts if you want to read up before your appointment.
If you can, get this helpful book: http://www.amazon.com/When-Sex-Hurts-Womans-Banishing/dp/0738213985. It describes a variety of conditions that cause chronic pain, their symptoms, and how to effectively talk about it with your doctor. Good luck!
I finally started using my dilators!! I got a new set instead of this one and it is so much better! The small size was something I could actually do :D and now I've moved up a level!!! First ever!!!
Just seeing it as a stretch like a calf or quad stretch helped change my mindset to it and not worry about having to feel turned on. Plus, I finally made a routine of setting up a towel and having lube handy.
Have you tried the Berman dilators? They vibrate and you can work your way up the sizes. They’re a four piece set and are about 20 dollars on walgreens.com
I also have this: https://www.amazon.com/dp/B000BIPK10/ref=cm_sw_r_cp_api_ATUYzbQ1VQ1PE and I like it a lot. It’s about an inch wide.
Penchant premium. I got it from amazon: Intimate Lubricants for Sensitive... https://www.amazon.com/dp/B00UO1R7XI?ref=ppx_pop_mob_ap_share
Its specifically for sensitivity, and it's $14.95.

I totally misspoke. Sorry. sigh I'm supposed to be an expert right now, right?
Here's the link on Amazon.
I think you really need to go talk to a gynecologist. In particular, the "deep inside" pain you are describing does not sound like vaginismus. You can also check out this book: https://www.amazon.com/dp/B001NJUPFW/
I read A Simple Guide to Vaginismus by Kenneth Kee recently and it suggests a connection between pain and vaginismus, doesn't have to be genital pain. Ignore the review, it's a very informative book.
According to the book any of those health issues you mention could have triggered it :/ No one really knows why.
