The two books I read were targeted towards ESL and ELA assessments, so they place a heavy emphasis on language choice in questions, as well as variety and usefulness of language choice in question formats. They might not be the most useful for maths, but why the hell not:

First one and most useful: Authentic Assessment for English Language Learners

This second one was used somewhat peripherally during my first grad program, and mostly for content development, but we often reflected on it while developing assessments for the materials we'd created based on the tenets within the book. Developing Materials for Language Teaching
