Yes, anything can have a dramatic impact on an individual.

From a total population standpoint is it normally a primary concern? No, most people do not even notice any issues that develop but then again, most people are not pushing their bodies or asking them to be high performance machines.

You are doing the right things by strengthening your muscular imbalances that have developed. Imbalance is one of the leading causes of injury behind only previous injury and severity of previous injury. My first recommendation would have been to get an assessment and to strengthen the weak areas. Since you have already tackled those issues, now you are also at the point of prevention.

What can you do and what's important?

What is important is the alignment of your hips and spine as you sleep. Prop your body up into a position where your hips and spine are aligned. I'll give some links on pillows that can help accomplish this.

Another common pillow position for back sleepers is a pillow, roll, or towel under the hamstrings.

If you have short hip flexors that put you in an anterior pelvic tilt (APT) this will especially work to take the pressure, from the weight and leverage of your legs, off of your low back.

She still has occasional pains and some days are better than others. She was prescribed opioids for the pain but refuses to take them for the obvious reasons. She was taking 4-6 aspirin every 6-8 hours but now she isn't taking any daily pain medication anymore and only takes a couple ibuprofen on "bad" days. We also recently started going to the gym where we do different muscle groups each visit but it seems like back and legs days have helped her immensely as well.

Yeah the nausea from meds is one reason I stopped. The nausea was worse than the minimal pain I had at that point.

unless you want to spend the money on a latex mattress (it's better for sex and it'll last longer than regular polyfoam), i'd recommend you go to www.foambymail.com and pick up 4-6" of HD foam (firm) for the base layer (the more you weigh, the more reason you should get 6" instead of 4"...the price difference is negligible) and then depending on your preference, get a super soft 2-3" top layer, medium 2-3" top layer or even a 2-3" memory foam (i'd recommend the gel memory foam, as regular memory foam sleeps hot but the gel helps keep you cool) if and only if you prefer the feel of memory foam.

as an aside, if you do wnat to buy latex, do NOT buy from foambymail (all their other foams are fine but the latex is not good quality!). i have three latex mattresses, all from brooklyn bedding/dreamfoam bedding and can't be happier. but these are $1500+ if you really want the benefits of latex and you have a very firm foam base layer already, you can buy a 2-3" medium topper for $300-500 (queen or king sized).
