I'm going to sound like a major downer, but planning for the worst case scenario now will allow you to relax and enjoy yourself when you're actually on vacation.

(1) Wear a medical alert bracelet, and carry some sort of wallet card with vacation specific information. Think about the info that a first responder would need to treat you in the event of a hypo or non-diabetes related emergency, and what information would let them get in touch with your family. Big, bold DIABETIC or INSULIN DEPENDENT on the bracelet/necklace/whatever-will-be-least-obstructive-to-having-fun, and the name of your cruise ship and the names of your companions on the card. Don't put a room number on it. Someone legit will go through the ship's staff to get in touch with your folks in an emergency; someone sleazy will steal your wallet and use your room number and key to steal your stuff.

(2) Bring way more pump supplies than you think you'll need. You're going to be sweating and swimming and doing other things that will cause the adhesive on your infusion set to give up much more quickly than it usually does. Also, the hotter the ambient temperature, the faster the insulin in your pump breaks down. One option is to get something like the Frio Pump Wallet. I have one that I use when I'm going to the beach or anywhere else where I'll be in the sun and heat for long periods of time. You just soak the inner compartment in water to activate it, so you don't have to worry about bringing ice packs with you or stashing your pump in someone's cooler while you sit awkwardly tethered to it. Another option is just to only fill your reservoir with what you'll use in a day (+20-50% considering you'll be eating cruise ship food.) Depending on the type of infusion set you use, you can just change out the reservoir in your pump and not change your site if it's still stuck in place and working fine. Also bring plenty of syringes just in case you do have a problem that makes you switch to MDI, and talk to your doctor now about what you should do to cover your basal rates. If you've taken something like Lantus or Levemir in the past and know how it affects you, this might be a good time to ask for a prescription for a single pen of the stuff and some pen needles to bring along just in case.

(3) Bring water-proof glucose. This may sound silly, but you don't want to be relying on a zip lock bag to make sure that your glucose tabs don't dissolve while you're scuba diving. I'd recommend something like Insta Glucose. It tastes like crap, but each tube is 24 grams of fast acting carbs, and if you have a low while you're out in the water, you need to bring it up as quickly as possible. I've also found that the packaging holds up better than Dex 4's glucose gel tubes. You can tuck one of these guys in your wetsuit or some sort of arm band and bring it in the ocean with you without the label running or the tube getting all funky.

(4) Are you going scuba diving off of a boat or off of a beach? If you'll be out on a boat and aren't bringing your pump for any of it, bring a vial of insulin and a syringe so that you can dose if you get out of the water and you're sky high instead of waiting until you're back on land. If you're going off of a beach, still bring a vial and syringe and put them in the locker with your pump. If you're high after the dive, the syringe will bring you back down more quickly than your pump would, especially if you're a bad ass and don't mind giving yourself an intramuscular injection.

(5) Cruise ship food will wreck you. Everything is super fatty (yes, even the salad) so expect longer absorption times for food. If you're comfortable with your pump's square bolus/combo bolus functionality, this is the time to use it.

(6) Sunblock. Both because sunburns are no fun and because the inflammation of a sunburn will cause your blood sugar to run high with no rhyme or reason.
