I found I was much more focused working on a whiteboard whenever possible. I got a cheap one for about $15, worked great.
Also web browser plugins that restrict website usage during work time, so I blocked facebook, most of reddit, twitter, things like that from 8am until 1pm and from 2pm until 5pm. Works great.
Also (and people are going to think I work for this company because I'm always advising everyone to try it) I bought this diary and followed their work principles to the letter (outlined in the first 10 or so pages), it's an amazing product:

Edit: Also, make sure you take a number of 10 minute during the day, step outside and just breath some fresh air.

I literally just bought a planner on Amazon two days ago... really kicking myself. I got this one which I think has a lot of the same features without being so cute. :( It just shipped so can't confirm.
