A book that really helped me with this was 8 Step Recovery: Using the Buddha's Teachings to Overcome Addiction.

The author also has these free online addictions meditations you can do. I recommend trying. Even if it feels silly, or it feels like you're not doing it right. What have you got to lose?

I know I'm far from clean but I'm chipping to keep a physical addiction away. I'm reading about and practicing mindfulness and meditation. I can get myself to a lovely centred and pecaful place where my depression doesn't bother me. If I can get to a point through practice and therapy where I can achieve that most of the time I will be ready to quit for good.

If that's your bag I really do recommend this.
