There is a company on Kickstarter developing a HRM that reads heart rate from your wrist, but it is not out yet. As I understand, most or all existing HRM's that do this are crap. But this is a completely new technology and it's been said that this is the future of heart rate monitors.

That being said, today you will want a chest strap model. You really do get used to it. Get one with a soft chest strap. Polar makes many nice models. If you enjoy outdoor running, you might consider getting a GPS watch with built in HRM. I was lucky enough to have a friend give me a Polar HRM similar to this one he was no longer using, and I used that until I decided I wanted a GPS, so I upgraded to a GPS/HRM watch with chest strap.

Garmin is probably your best bet for GPS/HRM. If you want just a straight up HRM, I can definitely recommend Polar.

Here is an older model Garmin, you can save money by buying last year's model. A good friend of mine has that model and he loves it. The Garmin Connect website is awesome too, you can upload your runs from the watch and track progress, routes, share with people, etc.
