Our little one is 7 weeks and we are coming out of something similar. I have found that she stays asleep and is generally less fussy if she has gripe water or gas drops a few times a day. I'm not a huge fan of medicine so I prefer the organic gripe water but our pediatrician said both are fine and will not negatively affect her.
Some gripe water or gas drops along with a tight swaddle seem to have solved our issue about 90% of the time. I realize all babies are different and everyone has a different opinion on giving the baby anything other than BM or formula, but if she's happy I'm happy.
Also: the swaddle me pods Link seemed to help with the hands waking her up when she wasn't swaddled. It gave her enough room to move them a bit but stopped her from flailing of hitting herself in the face.
Best of luck
If she breaks out of the Velcro swaddles, you could try something like this maybe

I have one on Amazon and one at Target. Things that I really like from previous babies are;
Also, not on my list is witch hazel spray/liquid/pads for your lady parts. Because they will make things SO MUCH BETTER. Also one of those squirty bottles to squirt on yourself when you pee. Because urine burns on swollen areas. I stole mine from the hospital. xD
Looooved the swaddle, I think it was a major reason why my first was such a good sleeper. I really like the SwaddleMe Pod for newborns and the SwaddleMe velcro swaddle for slightly older infants. It takes all the stress out of trying the swaddle.

Or you could try double swaddling and see if baby breaks out of that. Usually the double swaddle is harder to break out of
We also called our baby Houdini. He Could get out of the tightest swaddles (my husband is a pediatric nurse with 15 years swaddling experience). Ultimately the only swaddles that worked for us were these

I'm one week out and these are the things I've used the most:
I use the blankets and pods to double swaddle her. Even though the pods are newborn sized she can still get her hands through the neck as well as it bunching around the neck. (She was born 6lbs 15.6 oz) so we first swaddle her in the muslin and then zip her up in the pod. Since doing this she sleeps 2.5-4 hour stretches at night. (She regained her birthweight and some by 1 week so I don't have to wake her up to feed every 2 hours at night, yay!)
Things I'm not using yet
The ones he has say 5-12 lbs. They are pretty snug and he was 8.5 lbs at birth. Understood on not wanting to buy teeny stuff. I have a few onesies he grew out of in the womb!
