The Jetstream pen by uni-ball is exactly what you describe for tip #1.
I'm a righty but have a claw or hook to my hand when writing. What this does is that my right pinky and ring fingers would be dirty like this every day in school from "dragging" my pencil behind when writing.
The Jetstreams dry super quick and are amazingly smooth. Found them early on in college and still keep a supply of them handy. Might be a little pricey but for me, since I don't have to push as hard as a regular cheap pen to get clean solid lines, my hand doesn't cramp up as fast.
The Uniball Jetstream is the best pen I've ever used. Mark it, dude.

Been using it for years, never went bad on me. At $3 a pop it goes to show that you don't have to spend a fortune to get somethign outstanding.
Mont Blanc, Parker, Cross pens cost tons more but don't match its smoothness IMHO.

In addition to the Parker G2 style, another option is the rollerball & gel refills that are somewhat standard sized. Examples for them would be the Pentel Energel, Zebra Sarasa, Pilot G2, and Uni Signo 207/307. They're all relatively interchangeable. (I believe the Pilot G2 tends to be less compatible than the others.)
