Ever read this book? Women Don't Ask: Negotiation and the Gender Divide. It's some interesting research that confirms what Thankful_Lez is saying. It is a societal norm. Sure there are other causes... but our societal norms (which is a complex combination of different ideas) about gender are the root causes, i.e. the most important ones.
