You might look in to finding a rolfer. It's a great structural technique for balancing the body, with a strong focus on standing/sitting straight. They'll measure your imbalances, then work on them, re-measure, work on them again, give you tips on maintaining the balance, etc.

A back buddy could also help with the trigger points in your back.

Photos are difficult to show postural changes very well.

However, check out the book Rolfing for lots of photos.

Yes, posture can change, but for most people, it takes time to learn the exercises, time to retrain your nervous system. Outside help like a trainer or practitioner can help speed that up. Ida Rolf could do it in 10 one hour sessions. Otherwise you're looking at months or years of exercises. Don't get me wrong, I don't think Rolfing is a substitute for postural or mindful exercises, but it can dramatically speed up the process.

You could try using a chest expander (which is actually primarily a back exerciser). I picked up one recently to see if it could help me with my posture. I don't think the standard movement (pulling it apart in front of you across your chest) can be replicated with weights - reverse flies look similar, but the angles of resistance are different enough for the two to be separate exercises. Single-arm sidelying reverse flies lack the bilateral/standing factor, which makes them less effective for postural improvement IMO. I've only used mine for a few days, but so far I think I'm getting postural benefits out of it that I don't get from any weightlifting movements.

Check out a thoracic wedge. I use it to correct thoracic tightness from computer posture. Lay on it and take 5-10 breathes then move it an inch. Dont use it below ribs or above the shoulders. Keep your lumbar spine flat (ie dont arch to allow your ass to touch the ground) and use your legs to vary the pressure.

Not sure it'll correct your issue, but it may offer some relief.

A peanut to MFR the muscles around the spine may also help get you some relief.

Move around before the neurologist. See Time Magazine article, The Exercise Cure.

Move your arms in circles. Straight out to the side, they make a line. Use the mirror. Do small circles, medium, lard, back smaller, then micro. Extent your shoulders(means pull both hands away from center, a subtle stretch, or painful?
Do hip circles. Pelvic rocks. Do the twist.
Roll in the grass.

All this stuff.

It is just habits, young or old.

They sell ~$200 chairs for that. An under-$20 alternative is this air cushion which is great. Buy it, really it was $35 in the 1990's when I bought one which is more in today's dollars. I sat on it and recognized how it allowed me to subtly shift position with bottom support.

I was in a similar situation to yours. I have a maxnomic chair comparable to DXracer. I found the included pillows terrible. I tried multiple lumbar supports with no relief. What finally worked was a samsonite lumbar support I found at costco (little more expensive on amazon) because it had more squish to it. When I use this one, my shoulders/head don't make contact and I'm able to play FPS games for hours at a time without pain.

This seems to be the one. It was $10 at costco (I plan on going back for an additional for work)

Also, with my maxnomic my feet weren't planted on the ground properly. Consider trying some form of foot rest if that is the case for you.

I use monitor arms(https://www.amazon.com/dp/B00B419MZ8?psc=1 - note the longer pole than some) so I can change the height of the display throughout the day. I agree too, a higher monitor is better for me.

If you're hunched in normal use then get your monitor(s) raised to force yourself upright. Three packs of printer paper under a monitor is about right for a a tallish person.

If you're using a laptop then consider getting a nexstand or similar to get it raised. You can use this in a standing position easily too.

Sit on a Swiss Ball if you want to keep your core active while working...

If you're open to another option, a saddle stool is good at enforcing good posture too.

I've had mine a couple years and no weird smells and it's holding up great. It's really versatile too, I use it a lot when taking photos for my side business, it can raise up to almost standing height for me so I can easily go from standing to sitting or I can lower it to a table/desk height and use it that way. Plus, it lowers enough to be fully pushed under a table/desk so it saves space too.

I actually use these in conjunction with an ergonomic stand that straddles my body so that all my joints are in a neutral position. I look all sorts of ridiculous, but my vertebrae loves me for it.

Get a wedge pillow, it will allow you to keep your upper back from rounding and you can rest your head back comfortably.
