Here are the products that I used for this:

Note: if your phone already has wireless charging, you don't need the Mophie case. My phone is an iPhone 6S so I need a charging case but you won't need that if you have one of the new iPhones or Android phones that have wireless charging native to the phone.

Mophie JuicePack case (home charging pad included):

Mophie sells a car vent mount that you can use. However, I didn't use that because it doesn't swivel that well. So I bought the Mophie Desk Mount:

And I disconnected it from the stand...so I'm not using the stand at all.

Instead, I connected the charge pad (that used to be connected to the desk mount) and connected it to this:

So I disconnected the magnet on the WizGear vent mount and connected the Mophie charge pad to the WizGear.

The 17mm ball on the mount is the same the desktop charger uses, so I just took the wireless charger off the desk stand and attached it to the dash mount.
