Your first point about getting a good mic is so true! I've seen way too many videos with some low quality, noisy recording and it's just not worth listening to. For my videos I either use my Sennheiser headset mic (although I don't like how loud my breathing sounds with that) or my Samson C01U which sounds fantastic.
I'd also recommend cleaning it up in something like Audacity to remove any static in the background.
My friend has a Turtle Beach, and he's thinking about selling it because it's just so difficult to use. The microphone always plays back through his headphones, and it uses up a USB port as well as the headphone jack.
I bought a Sennheiser PC330 G4ME for about £60, but for some reason it's more expensive in America. However, it's definitely worth the money.

Amazing headphones for games. Not costly at all but have fantastic range and are incredibly comfortable to wear. I have yet to see any headphones near the HD518's that are equally as good. I have worn these for hours on end day after day.

Tried to get into the whole "Headset" deal. Was not impressed, the earcups on the 330's are aweful compared to the HD series. I really should have gotten the next level up for the better earcups. The microphone is decent quality and comes through clear without any interference. So that's a plus.

Probably my oldest pair of headphones. Got these guys when they were hip. They are a fantastic set and are excellent to listen to all day, Although the earcups are kinda "flat" compared to the others. So I can feel the earcups against me. It creates a better listening environment but after long sessions you may get irratation on the sides of your ears
After Boxing day I will probably be hunting for another sale. The Sennhieser Game Ones use the same HD series earcup, The new corsairs Voids are out which I would love to try. Of course Audio-technica's are always really pretty.
This is your best bet. Ignore the USB part of your request because any headset can be made USB for very cheap DAC's.
The majority of USB headsets ship with a low quality DAC anyway. So don't bother.
Grab one of these as well if you require USB.
That's a very high quality one, but you can go cheap if budget is an issue, and then upgrade later.
Truthfully, one of those has to get cut.
Reliability is very subjective, most "gaming" headsets don't last long. If I were to recommend something reliable to you, I would recommend the PC330's, the only thing they lack that you want is a USB connection, which means nothing. It is actually worse. There are USB sound cards for 2 dollars out there, or you can get a regular sound card for your PC that is 25 dollars (Asus Xonar DG), these are much better.
