I'm 24 and just had my first orgasm a few months ago! I had the exact same problem you do, never realized it was a fear thing but that's actually a good way to describe it.
The times I got close, either by myself or with a partner, I would feel a somewhat-good buildup, followed by increasing discomfort, until I had to stop whatever stimulation.
