A raspberryPi running raspbmc.
EDIT:
Here's everything you need. And you don't need internet access for anything to function.
RaspberryPi (Included power cable and case) $56
SDcard preloaded with RaspBMC $25
Handy little Remote Control (that you can use as a keyboard in a pinch) $7
Good short HDMI Cord $7
this is what you're looking for
or another idea,
Buy this modify/dremel it so you can somehow flushmount it into your desk.
You'd have to make it removable to replace battery every few months, or you could find out what voltage the battery is and hardwire from a molex connector from your PSU, cutting down the voltage using resistors to get the correct voltage to the controller to eliminate the need of a battery.

Took me all of 4 seconds to find. ;p
I used this remote with my pi and kodi: https://www.amazon.com/SANOXY-Wireless-Remote-Control-Mouse/dp/B0050PUGZE
Worked fairly well, although had to make sure the IR sensor was visible.
Someone in another thread confirmed that this cheap one works great with Harmony remotes:

You program it as 'ChinaVasion CVSB-983' in the Harmony settings. Not sure if that's supposed to be 'Vision' instead of 'Vasion' though.
One of these. Its in harmony as a chinavision or something

I use EmulationStation/Retropie as my O/S, and launch Kodi from inside that.
I have a cheapo controller (like this one, but I paid half this price) and it works great with the adapter plugged into a USB: https://www.amazon.com/SANOXY-Wireless-Remote-Control-Mouse/dp/B0050PUGZE/ref=pd_sim_147_3?_encoding=UTF8&pd_rd_i=B0050PUGZE&pd_rd_r=CBXHJVNC0C7QEF6CZ8WA&pd_rd_w=m6UWn&pd_rd_wg=0rrB7&psc=1&refRID=CBXHJVNC0C7QEF6CZ8WA
Why aren't connecting a USB keyboard to type? I'm sorry I can't help with the controller question... good luck!
for the remote, are you talking about something like this ?
Thanks for the build!
