A tiny bit over your budget, but one of these things are invaluable to paramedics/EMTs/cops.
I was a cop and a paramedic a few years back and it was my most treasured item on the raod.
Edit: I just noticed that the one I linked was only 6x10 so you don't want to get that one. The 9x12 is the one that you'd want to get, but that is $19.99 so it may be too spendy for ya.

i had to look it up, but apparently its called the adams form holder. they do make a couple variations, including one that fits a full unfolded 8.5x11" sheet, but i use the 5.4x9.5" size that i linked for you.
