My first book was a kit, the inside block was pre-made, and everything but the paper pre-cut. You can get the kit here. The instructions are here.
If you want to try a simpler notebook (no gluing), this is a good starting set of instructions.
This page has a very detailed step-by-step, but I would have been lost my first time. Building the block (the inside part) is a bit involved sometimes. This is a great step-by-step, but you'll need some tools beyond a knife and glue, so I'd recommended getting a pre-made block and making the cover the first time, just to minimize your investment. You can just buy blocks here. (I'll note that two links have gone to Hollander's; their my local store, so you can Google around for others, but they're usually pretty reasonable. Shop local if you can to skip shipping.)
If you like it and want to try other styles, I've been working my way through this book lately. It has good pictures and instructions for all kinds of styles (accordion books, Japanese stitches, etc.).
