My shih tsu, Frodo, had the same problem which my vet said was due to his seasonal allergies (itching, breaking the skin causing tiny sores which would then cause him to itch more and sores would get infected... a vicious cycle). I would get him a shot to control the itching twice a year and have temporary relief from the smell until the perpetual itching would start again.

It was my sister (who is a veterinarian) who was visiting one year who finally gave me the solution that worked. Frodo and our family suffered with his itching and his smell for over 8 years. He was not allowed on furniture or beds bc of the smell. His last 9 years were smell free & itch free, hallelujah! Her recommendation worked so well that he never needed to get the shots anymore.

My sister's recommendations:

supplement his diet with omega 3 capsules or oil

wash him with an antibacterial shampoo when he is itching to control sores getting infected (I use NaturVet Septiderm-V Skin Care... https://www.amazon.com/dp/B0006IC5QI?ref=ppx_pop_mob_ap_share )

after shampoo, rinse with a leave in conditioner (I use Epi-Soothe Cream... https://www.amazon.com/dp/B0056EAJZQ?ref=ppx_pop_mob_ap_share )

when not itching and for regular shampoo, use an oatmeal shampoo followed by the creme rinse
