Absolutely get a food scale. This is the one that I bought: www.amazon.com/gp/product/B005OSUIYG/
For the rare or odd homemade thing, the Law of Large Numbers applies. When you do one of those generic "homemade" searches, don't always pick the lightest one or the heaviest one. Pick the one that is just a little heavier than the average, just to be on the safe side. It'll be close enough and the Law of Large Numbers will eventually take care of you.
For the frequently eaten homemade thing, learn to put in the actual recipe into MyFitnessPal and log a portion. You'll have it there for the future.
Learn how to copy yesterday's breakfast or last Tuesday's lunch into today. Saves a lot of time.
Learn how to Quick Add Calories -- I did it last night to add 100 calories to make up for the buttery-looking but mystery-containing goodness that was on my restaurant broiled salmon last night.
Almost all packaged foods have serving size both in "Standard" units (which vary - most things are by volume) and in metric, which is uniformly either ml or g.
My bet is you're eating more than you think.
Buy a little digital scale. I've been happy with https://www.amazon.com/dp/B005OSUIYG .
Weigh everything and track what you're eating.
For drinks, if you go with "1g = 1 ml" you'll be close enough, because that's true for water, and most drinks are mostly water.
I've been using this one for well over a year now and it definitely gets the job done. It's really simple to use and you can switch to ounces, grams, kg, lbs and use tare. I'm thinking of upgrading to a more sensitive scale though - this one tends to not pick up on really small measurements if something large is on top, like when my ceramic bowl is filled with a huge smoothie and I want to sprinkle chia seeds on top.
Otherwise it's great though - you can put your blender on top of it and weigh and tare the ingredients as you go.
I feel for you, and it sounds really rough. The good news is weightloss is 90% food and 10% exercise, so you can totally do this!!
I don't know if there are any digital scales without auto shutoff. I have an Epica accupro scale that shuts off after 3 minutes.
It was $14.95 when I purchased it 4 years ago. Not sure why the price has gone up now, though I've noticed Amazon's pricing can change from day to day.
Non-mobile: this one

I'll second the suggestion for a food scale, they're relatively inexpensive so it's a worthwhile investment.
Food scales: this is what I use, this is what Keto Connect uses and this is what I bought for a friend of mine.
Some users here prefer the cronometer app so they can also track their micronutrients and is worth checking out.
I use this one.

Best investment I ever made: http://www.amazon.com/Epica-Accupro-Digital-Capacity-Chrome-Electronic/dp/B005OSUIYG/ref=sr_1_13?s=kitchen&ie=UTF8&qid=1413919975&sr=1-13
I have one at work and one at home. It's a total game changer.
I have this one. Also 11lb capacity. Only $9.95 (with Prime). Had it for a little over a year now and use it nearly every day.
My food scale is invaluable for tracking calories. I use this one from Amazon. I have had it for almost five years.
