Our dog did this a few times because he’s in between sizes, what we’ve done for about 4 months now is use a lead with two clasps on either end and attached one to his collar and one to his harness so it creates a handle/handbag shape, that way if your pup slips the harness they’re still attached by the collar or vice versa. No problems with him slipping it since! It also takes a lot of wait off of the dogs torso/collar as it’s split between two different parts of the lead and it’s a fantastic technique for dogs who don’t fit harnesses without slipping them.
