Project Diva Dreamy Theater 2nd is an HD version of Project Diva 2nd while F2nd is its own game with some from Project Diva 2nd and a couple new ones.

They do not have the same graphics. Dreamy Theater 2nd looks like this while F2nd uses the same engine F uses but a looks little bit better.

Project Diva Dreamy Theater 2nd runs at 60fps while F2nd runs at 30fps

Dreamy Theater 2nd has 46 songs (all from Project Diva 2nd) and 31 songs from Diva 1 as a bonus.
Project Diva F 2nd has 40 songs and with all the dlc songs, it goes up to 56.

Project Diva Extend is a PSP game like Project Diva 1 and 2nd. Dreamy Theater Extend is an HD version of it just like Dreamy Theater 2nd.

Dreamy Theater 2nd is only available on the Japanese Playstation Store. You need to have Project Diva 2nd because you are required to transfer save data in order for the game to work. It costs ¥3900 which is equivalent to about $40. You can buy a ¥5,000 PSN card from Play Asia, make a Japanese PSN account and buy the game. That's what I did to get the game. It's really easy to make a JP account.

I also doubt it comes with the DLC, as it seems to be just a code, and not a physical copy, and there's nothing on the website that mentions it.
Amazon says the release date is December 31st, 2014
