Prolactin is what causes the "refractory period" in males. High chances that it's what's causing the problem.

High prolactin can lower testosterone too, which would explain the healthy gonadotropin and low test levels.

I'd start with P5P or B6 (This stuff has both) You want pyridoxine hydrochloride, not pyridoxal hydrochloride. Try it for a few weeks rather than caber. It's going to have fewer side effects and should get things moving in the right direction. If there's no change after 2 weeks, start the caber.
