I setup and produced a setup similar to this. Here's what I used on a budget to get pretty fantastic quality IMO.

Two Sony A6000 ~$600 each

Roland V-1HD video swithcher ~$974

Elgato HD60s capture card ~$160

That's ~$2,600 but you'll still need a decently spec'd laptop, camera mounts, cables, etc.

I also used a Atomos Ninja Flame and a Teradek Vidiu Pro, but those are unnecesasry for the end goal. The Ninja Flame is great for back up recording and real time color-correction, but also is an extra $700 or so...

The other issue is that even if OP can find a device that will split the image in that way, it's likely to introduce lag -- which isn't a problem for video playback but will make gaming feel clunky. An HDMI video mixer would probably do it, but it's going to be very expensive.

I do this, but I have a completely different setup. Unfortunately, my setup does cost a bit more, but it works flawlessly.

Basically, I bought an internal PCI capture card that I put in my PC (mine was the Avermedia Live Gamer HD, but I recently just upgraded to the Live Gamer 4k). I plug the PS4 into the input HDMI port on the capture card and run the output to my TV/Monitor (depending on what I am playing on). I then add the "Video Capture Device" source in OBS and set the audio to output to the desktop. With the sound output to desktop, I can plug my headphones into my PC and hear both OBS sounds (like alerts, etc.) and my PS4 audio (including PS4 party chat output).

I use a Blue Yeti USB mic plugged into my PC to talk to my audience while I stream. I then bought a cheaper Blue Snowball to plug into the PS4 to talk to my party chat (love the fact that PS4 lets you use a USB mic as an audio input source without anything extra installed). Now, my audience can hear me and my chat can hear me without any loss or problems. I just plugged the Blue Snowball USB mic into one of the front PS4 ports.

One word of warning though with PS4 and USB mics. I tried to use my Blue Yeti mic connected to the PS4 and I lost my PS4 audio while it was plugged in. The reason is the Blue Yeti has a 3.5 mm audio jack built into to allow you to plug headphones directly into the mic itself. For some reason, the PS4 defaults all audio output to that little jack and I have not found a way to change that through the PS4.

Anyway, I know it is a more expensive setup, but it has always worked flawlessly for me and lets you know it can be done. Added bonus is the capture card lets me stream almost anything with HDMI out, like my PS4, XBONE, Switch, and my gaming PC.

EDIT: I just noticed that you do either something like this or this exact setup already. As far as I know, the only way to get true stereo would be something like a mixer, but not sure if they make one that will handle this.

EDIT 2: Maybe something like this, but it is expensive.

Sorry for the late reply friend. afaik the only ways to make use of live feed function in PP6 is to use one of Renewed Vision's recommended adapters for video capture.(link). What I ended up doing, however, was making use of the chroma key function on my video switcher that I didn't even know was there at first lol. I just made sure I had my default background in PP6 was set to black and poof everything worked seamlessly. Best part was there was no added lag to account for. My switcher does pretty well with latency and there is no noticeable delay for live video at the front of house. Now I know you said your switcher is in another room, but is it controlled physically by someone else, or is it software operated? In any case, I'd say it'd be worthwhile to see if your switcher has a chroma key feature, it would make your life alot easier, considering your already rather involved setup. imo it's really worth it to consider buying a switcher with at least the chroma key feature. It means less clutter and an overall more professional look, just my two cents. Here is my current video switcher if you're interested.
