Have you tried an electric foot warming pocket like this one? I've had a bit of the same problem, but not as bad, and was thinking about getting one. It can be used in bed or seated, and the reviews look promising.
I've been getting by wearing warm socks all the time like you, even in bed, and warm Acorn slippers. I especially like these Acorn Versafit fleece socks, they're as warm for me as thick wool ones but not as bulky, so I can wear them with slippers, and they have flat, non-rubbing seams for safety. Be careful ordering the size, though, they're not stretchy like normal socks, more like felt booties.
