You won't get anything this complicated without a custom build. You could either mod your mixer, or build a custom enclosure/case to house it all (i recommend the latter)
Otherwise, If you don't need the functionality of a full mixer, and just want more controls over your audio... you could harvest the parts from a headphone amp like the Behringer amp800 and integrate it into your case. I use this little amp at home so that I can have multiple inputs and switch between sources easily.

If is not clear if the Original Poster has resolved his problem or not, nor is it clear how much money he is willing to spend to solve his problem.
While looking for something else, I stumbled across this 4 Output Headphone amp, but more importantly, this headphone amp has TWO STEREO INPUTS with separate volume and balance for each of those inputs. So it seems it will mix to input channels and feed those to the output of 4 individually controlled Headphone Outs -

You might want to look a little deeper into it, and if you can afford $70, that would seem to be a solution to your problem as I understand it.
