Don't you see the black vertical line in each handgrip? That's an insulator. The left half of the handgrip is electrode 1, the right half of the handgrip is electrode 2, and they are insulated from each other by the black plastic.
The shock comes from the secondary of a transformer, whose primary is stimulated by a small and well-controlled AC waveform, generated by a battery and a little (gasp!) circuit. The transformer secondary drives electrode 1 and electrode 2.
It's the exact same electronics as you find in Example_1 and Example_2 and Example_3 and Example_4. Haven't you taken one of these apart and studied their guts? ALL the EEs in my university did this when they were little kids consumed by curiosity.
Maybe 5 years ago I had a few friends ask me if I could suggest any books on FIRE since I was closing in on my RE date and they knew it.
