Our dog did this a few times because he’s in between sizes, what we’ve done for about 4 months now is use a lead with two clasps on either end and attached one to his collar and one to his harness so it creates a handle/handbag shape, that way if your pup slips the harness they’re still attached by the collar or vice versa. No problems with him slipping it since! It also takes a lot of wait off of the dogs torso/collar as it’s split between two different parts of the lead and it’s a fantastic technique for dogs who don’t fit harnesses without slipping them. I’ve popped a link for the lead we use so you can better visualise the set up.

Doesn't look like you can buy the adjuster clip by itself. My guess is that you need a piece of special equipment to attach the end of the cord into the fixed end of the loop, so they don't sell it for home use.

Another vote for a gentle leader. I have 160 lb puller (with a severe hatred for squirrels) and the gentle leader along with one of these leashes makes walks an absolute joy.

It was probably a long corded leash like this. Retractable leashes are very thin, and when the dog runs and pulls while it is wrapped around a limb, I imagine it can cause serious injuries. Plus the owner would get rope burn in attempt to control it because the leash has very poor grip. Very dangerous and impractical.

I know you've tried the "stop and go" method, you said as much. But I think you've not gone so far as to be a "walking pole". Meaning "we can walk nicely, but if you get ahead of me, I'm not moving". Plant your feet, and quite literally, don't allow the pulling to wherever she wants to be. This also presents a neat bit of training that looks so very impressive to the general public; "Heel". Every time the dog gets in front of you, and it's going to be quite a lot, stop walking, plant your feet, and gently guide your dog to where you want them, usually at your heel, give the dog the command when they get there, and give them treats. Keep this up for awhile, and eventually, when the dog gets ahead of you, and starts pulling, you'll stop walking, and your dog will automatically "return to heel". When they've mastered the 'return', you work in a 'sit' at your heel. This will effectively modify the "heel" command. They don't get the treats until they're sitting at your heel.

The other thing I'm seeing is, according to your post, you went from your yard directly into a high distraction area. Your yard is a low, or no, distraction area, because it's familiar to your dog. But you went from there to a place where every tree, person, dog, and blade of grass needs to be sniffed, like right now. Work up to that point instead in smaller steps.

There is one more thing I'm going to suggest. A very, very long lead (this is the one I have). Just hook your dog up to it, work on recall in high distractions (other dogs, people, etc) at random distances. The other good thing about this leash, in particular, is it gives the dog time to just go out to the park, and be a dog. If we were to stop and think about how much stress we put on our dogs with all the training, we'd think that there isn't a whole lot of time off. Dogs, like humans, need time off. Just go to the park, hook 'em to the long lead, and let 'em go wherever the hell they want. Then practice recall, treat, and let 'em go again. Dogs need time to go out and be dogs, lest they forget how to dog.
