I'm fairly certain it is a Barclay Products Model 4604. I don't think its a Barclay problem though. There appears to be 2 separate diverters on this setup, one being the shower/bath lever, the other being the Delta Style Push/Pull on the handheld connection. Can't really tell without a picture a little higher up, but I assume it also has a riser to a regular shower head which is also leaking.

Someone else please chime in if I'm wrong, but I think if you replace the Push/Pull Delta 50650, you should be good.
