Make sure it has enough buckwheat to support your head and neck and kind of bunch it up towards your headboard. You'll be in a bit of an incline (but comfortably supported) and your shoulder should be largely relieved of the pressure.
i use a facial mask and spend some part of my night face down in the "military crawl" sort of position. I have been a longtime user of buckwheat hull pillows.. Buckwheat hull pillows are far superior to a standard pillow in my opinion. They are firm and somewhat "malleable". also cooler and heavier. they can be easily smooshed to a good position for side of the head support while lying face down allowing room for the "apparatus" . sometimes I use two, one either side of the mask.
