This one definitely fits the bill. I have been carrying a similar one for years. The one piece all metal construction should outlast any fabric or leather one.

I use this because I must keep one ID visible at all times while at work, and I use the clip to attach it to my pocket or a lanyard. It holds 5 ID/credit cards easily.
EDIT: IF it 5 is too loose and 6 too tight(which it usually is), make it 5 and fold 1 or 2 pieces of paper(a $20 does fine) in the middle
