I actually just threw in some heel pads like these. It's not the prettiest thing in the world, but it's working really well right now. I'm hoping that once the boots have broken in fully that the instep will have collapsed a bit and I won't need them anymore.
I did a combination of three things, and it has worked wonders. I changed my lacing pattern according to this video. Instead of more expensive pads, I just bought whatever version of this they had at my local target (I think I paid <$5). And finally, I untucked the tongue. Once I hit on the combination, it was instant and complete relief. I haven't had even the smallest pain since.
