So currently you have 4 DVI out to 4 monitors and want to add a 5th that is a mirror of one of those 4 but are out of video outputs. Since HDMI accepts DVI-D signaling a normal splitter cable should work fine to the TV and monitor. This will break HDCP though (i.e. you won't be able to watch say Netflix on the TV and monitor using this output), if you need HDCP you can find a proper duplicator for cheap but this may add a small amount of latency. If your monitor supports HDMI it would be cheaper to get an HDMI duplicator (~15 dollars) and pipe the GPU output into that.

Best option if your case has room though is get a really cheap card with a few video outputs (perf won't matter as it's just copying streams) and you won't need to worry about adapters or latency or hdcp.
