For Q1 /u/wallawalla I would not state anything as it as "Special in-flight arrangements needed". /u/320A needs to just pick up something similar to a Frio insulin wallet.

The other questions I agree with, just put N/A.

For travelling make sure your insulin and medicines / syringes are in you travel bag that is with you on the plane (not in the hold luggage). You would be best getting a doctors letter to state you need them, but I have never been asked for this (I flew from Madrid to Salvador / Rio without issues).

Basically you shouldn't need help with anything. The staff might offer to refrigerate your insulin on the flight but I personally keep this with me, I like to keep control of it.
