Wish we had one, but we managed okay with a birthing class, a great supportive birthing team (my wife's sisters), and prepping by reading the best birth.

I highly recommend reading, The Best Birth. It's good for you to know especially. It'll give you a rundown of what might happen, and actually does a fairly good job of coaching the father to be involved as much as possible. When your wife is in that much pain, you will have to make some decisions, or know what the hell is going on so you can make sure her decisions are executed. The book takes the point of view that if you're dead set at only having a vaginal birth no matter what, you're not going to know enough about what might need to happen, and you might heel in when it's the wrong decision. It's a good counter-point to natural-only birth plans. I know too many women that wanted natural only, and once the pain started changed their minds completely.
Good luck!
